New Patient Referral Program

The referral of your friends and family is the greatest compliment you can give us! If you refer a qualified new patient* to Forbes Dental Care, you can choose one of the following rewards items as a token of our appreciation:

REFERRAL REWARDS:

• Sonicare Elite Toothbrush
• $50 gift card
• $50 credit towards dental work

New patients referred to Forbes Dental Care using the referral card will be entered into a monthly drawing to receive a free Sonicare Elite Toothbrush.

To ensure you receive a reward item for qualified new patient referrals, please note that the following must occur:

• Provide the new patient with a completed referral card.
• The new patient must reference the referral card and your name when scheduling their first appointment.
• The new patient must present the card to our staff when checking in at their first appointment.

TERMS & CONDITIONS: *Referral patient (new Forbes Dental Care patient that has not been seen at our practice in the last 36 months) must present completed card at time of first appointment for referrer (current Forbes Dental Care patient) to receive a referral reward. Rewards will be given after new patient’s first appointment. Referral credit can be applied to any dental service or procedure at Forbes Dental Care. However, it may not be applied to previously paid-for services or CareCredit transactions.  Rewards are non-transferable. Must be 18 years or older to receive referral rewards. This program is subject to change and can be discontinued at any time. Federal Law prohibits Forbes Dental Care from offering this promotion (referral reward/drawing entry) to anyone who is receiving services as beneficiaries under Medicare or state health programs.
